Below are the instructions on filling out the Free and Reduced Food Service Application.  Fill out the application, print and sign.  Mail the application to : Pollock Pines ESD, Attn: Sue Meeth, 2701 Amber Trail, Pollock Pines, CA  95726.

National School Lunch and Breakfast Program

The Pollock Pines Elementary School District takes part in the National School Lunch and School Breakfast Programs.  Meals are served every school day.  All of our meals meet state and federal nutritional guidelines. Students may buy lunch for $3.00, and/or breakfast for $1.75.  Eligible students may receive meals free or at a reduced price of $0.40 for lunch and/or $0.30 for breakfast.  Students may buy white or chocolate milk for $0.50. Adult breakfast is $2.75, and adult lunch is $4.00.  Please see the application above for specific information on the program and how to qualify.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by:
